Transition and Move Occupancy Oversight Consulting Services (379 beds)
TIMELINE: 18 months

Details:
5THink used an alternate service delivery model on this special project. The UCI project had a highly talented and skilled transition manager who had acquired a significant degree of understanding of the transition/move occupancy process, so 5THink provided an ongoing assessment of the transition process being delivered throughout the project. During our regular client meetings, we made recommendations to enhance the progress and processes that were put in place; we oversaw key deliverables and milestones; and ultimately, we produced and delivered our own tools for the facility's use when it became clear they were needed.

Challenges:
A challenge to 5THink was to work within the existing Transition structure developed by the client, as opposed to the 5THink project structure we normally put in place. Both sides worked diligently to blend the work approaches toward the resolution. Additionally, move guidance needs proved to be larger than expected, so 5THink increased our work effort and adjusted our work plans accordingly.

Results:
Monthly face-to-face work sessions and shared work documents achieved the mutual planning effectiveness sought by the organization. The structure/products that UCI took directly from 5THink's process were the organization and education of a Move Binder for each departments/nursing unit relocating, and also a "Mock Move" exercise. These were produced and delivered directly by 5THink and deemed highly successful in providing the select support and guidance desired in these areas.
